The Missouri Department of Health and Senior Services is seeking a contractor to provide HIV case management services in the Southwest Region of Missouri. The contractor will be responsible for providing services to individuals living with HIV/AIDS, including assistance with medical, social, community, legal, and financial services. The contractor must have experience working with community health partners, conducting stakeholder interviews, and managing projects related to chronic diseases. The contract period is from the effective date of the contract to March 31, 2027. • The contractor must collaborate with Missouri Ryan White grant recipients and other case management contractors to coordinate services and avoid duplication of efforts. • The contractor must provide coverage for staff assigned to HIV case management duties in the event of planned or unplanned leave. • The contractor must complete all trainings required by the state agency. • The contractor must ensure that new and rehired personnel are screened for Tuberculosis (TB) and complete a consent form for a criminal background check. • The contractor must maintain a personnel file for each employee, including complete and current criminal record background checks, resumes, degrees, and training records.
