The Department of Veterans Affairs is seeking a supplier to provide H-Wave H4 Multi-Function Electrical Stimulation Devices. The devices will be used to support Veteran recovery after surgery, restore function, and manage chronic or post-operative pain. The contractor will be required to provide bi-monthly shipments of the devices to the Consolidated Prosthetics Unit (CPU) in Bonham, Texas. The contract will be a firm fixed price contract with a base year and one option year. The supplier will be required to provide a price list, including the most current information regarding product features, and to maintain sufficient stock to ensure prompt delivery. Some key features of the H-Wave H4 device include: • Electrical Nerve Stimulator for Pain Relief • Provides H-Wave form treatment which induces not-fatiguing muscle contracts and positively affects nerve function, blood and lymph flow • Rechargeable Ni-MH battery • Two channels with two sets of lead wires • Timer with a range of 0 to 60 minutes • Digital LED display with button option selection for frequency selection from 1 to 70Hz • Dimensions of 7" x 4.5" x 1.5" • Weight of 1.5 to 2 lbs. • Consumables include a carry case, 2oz aloe gel, and 4 packs of electrodes The contractor will be responsible for delivering the devices to the CPU on a bi-monthly basis, and for providing warranty and maintenance services as needed.
