SUNY Downstate Health Sciences University (“SUNY-DHSU”) solicits proposals from qualified vendors of which one vendor will be selected to provide a remote hosted enterprise-wide Electronic Medical Record (“EMR”) Solution for use across all SUNY-DHSU facilities. The solution, as further outlined in the Request for Proposal (“RFP”), will be utilized across all SUNY-DHSU sites, University Hospital at Downstate and two satellite clinics. Those wishing to participate in this procurement may download the RFP documents from the “Documents” tab, and must submit via email a scanned copy of the Summary Information Form (Intend to Bid/No Bid) as soon as possible to Alicia Saleh, Alicia.Saleh@downstate.edu. If you are unable to locate the RFP documents, please reach out to request copies. PLEASE NOTE: Vendors must download and complete the following Exhibits and submit them with their written proposal: · Exhibit B: Mandatory, Weighted, and Preferable Requirements and Detailed Specifications, and o Please note that there are five separate spreadsheets within Exhibit B that must be completed: (1) Mandatory Pass/Fail Requirements; (2) System Functionality Requirements; (3) Technical Requirements; (4) Implementation Requirements; and (5) Preferable Additional Specifications. o Proposers must provide detailed responses and explanations to each requirement in their written proposal. · Exhibit C: Cost Proposal Form/Price Page. o Please note that there are five separate spreadsheets within Exhibit C that must be completed: (1) Cost Proposal for Anticipated Ten (10) Year Contract Term; (2) Hourly Professional Service Costs for Upgrades, Enhancements and Additional Configurations; (3) Renewal Options Cost Proposal (two five-year options); (4) Renewal Option #1 Hourly Professional Service Costs for Upgrades, Enhancements, and Additional Costs; and (5) Renewal Option #2 Hourly Professional Service Costs. · Exhibit G: Questions and Answers Form o Any questions or clarifications needed must be listed on this form and submitted to Alicia.Saleh@downstate.edu by the deadline indicated in the “Key Events” section of the Summary Information Form (Page 1 of RFP). Any updates to the solicitation and answers to questions will be posted and released through the New York State Contract Reporter, and via email. It is the responsibility of all vendors to: Register with the New York State Contract Reporter and sign up to receive notifications about this solicitation; or Periodically check the New York State Contract Reporter for any updates or answers to questions pertaining to this solicitation. This procurement is subject to NYS Finance Law Section 139 j&k, also known as the Lobbying Law. Therefore, vendors are cautioned that the only permissible contacts for this procurement are the individuals listed below. Failure to comply with this requirement can range from no contract award to debarment from sales to any agency.
