The Illinois Department of Corrections is seeking a qualified and licensed vendor to provide basic burial services for individuals in custody who are wards of the state. The contract will be for a period of 3 years, with an estimated 3 services per year. The vendor will be responsible for providing all labor, tools, parts, materials, instrumentation, and equipment to provide basic burial services, including transportation of remains to funeral home, transportation of remains to cemetery, grave plot for burial, receptacle, and digging of grave. The vendor must comply with all state, federal, and local laws. • The vendor must provide a record of services rendered, including the name of the individual, services completed, material and labor used, and location of burial. • The vendor must ensure that all pick-ups are coordinated with the facility's Business Administrator or designee. • The vendor must not leave their vehicle unattended with the motor running or the key in the ignition at any time while on institutional grounds. • The vendor must comply with all applicable Federal, State, and local laws, rules, ordinances, regulations, orders, Federal circulars, and all license and permit requirements in the performance of this contract.
