Description:
The Administration for Community Living is seeking organizations to implement evidence-based falls prevention programs for older adults with behavioral health and/or chronic conditions. The project aims to develop capacity to increase the number of older adults and adults with disabilities who participate in evidence-based falls prevention programs, empower them to reduce falls and/or their risk of falls, and create resources for a multi-faceted strategy to reach the target population. The program will be person-centered, trauma-informed, and require the awardee to develop partnerships with organizations that serve the target population. • The program will focus on evidence-based falls prevention programs. • The awardee will be required to develop partnerships with organizations that serve the target population. • The program will be person-centered and trauma-informed. • The awardee will be required to create resources for a multi-faceted strategy to reach the target population. • The program will aim to empower older adults and adults with disabilities to reduce falls and/or their risk of falls.
