The Colorado Department of Public Health and Environment (CDPHE) is seeking a technical assistance provider to support health systems in implementing evidence-informed strategies to increase hypertension management rates and support chronic disease prevention and management. The selected provider will work with CDPHE-approved clinics to implement practice facilitation and practice transformation, data quality improvement, and health information technology utilization. The goal of this project is to improve capacity and support sustainable implementation of chronic disease prevention and management efforts across Colorado. • The provider will support health systems to implement evidence-informed strategies to increase hypertension management rates and support chronic disease prevention and management. • The provider will report progress on activities and provide quantitative and qualitative data to CDPHE. • The provider will work with CDPHE-approved clinics to implement practice facilitation and practice transformation, data quality improvement, and health information technology utilization.
