• Pilot program to provide nursing assessment, intervention, and service coordination for high-risk pregnant & postpartum individuals and their infants through the first year after birth • Total funding available: $2,106,000 - Sangamon County: Up to 1 award ($351,000) - St. Clair County: Up to 2 awards ($702,000) - Winnebago County: Up to 1 award ($1,053,000) • Key requirements: - Must provide home nursing visits monthly during pregnancy and first 3 months postpartum - Maximum ratio of 1 RN/APRN per 50 enrolled families - Must have physical presence in service area within 30 days of start - Services limited to medically high-risk families only (not low-risk) - Must demonstrate experience in maternal/child health nursing services • Program goals: - Reduce maternal and infant morbidity/mortality rates - Address racial/ethnic disparities in outcomes - Provide screenings, referrals, service coordination, and diagnosis-specific education - Support identification and treatment of maternal mental health and substance use issues • Grant period: 7/1/25 - 6/30/26 with possibility of two 1-year renewals based on performance • Eligible applicants include public/private organizations, hospitals, community organizations, FQHCs, and nonprofits
