The Iowa Department of Health and Human Services is seeking a technical assistance provider to support rural Iowa service providers in achieving success in the Communities of Care Projects of Iowa's Rural Health Transformation Grant. The provider will offer expertise, assessment, and evaluation, and technical assistance on evidence-based methods for co-location, cost sharing, patient flow optimization, and compliance with evidence-based programming. The project will implement a portion of the Communities of Care initiative, funded through the Centers for Medicare & Medicaid Services' Rural Health Transformation Program. Key responsibilities include providing group and individual technical assistance, co-location planning, integration of non-clinical services, and chronic disease prevention and management.
