Description:
The State of Nevada, Department of Health and Human Services, is seeking proposals from qualified Medicare Advantage (MA) Plans to provide a Coordination Only Dual Special Needs Plan (CO D-SNP) program for certain Medicare and Medicaid eligible recipients. The CO D-SNP program aims to provide care coordination activities for Medicare and Medicaid dual eligible population, ensuring enrollees receive quality services and enhanced coordination for their Medicaid services. The contract term is five years, effective January 1, 2026, through December 31, 2030, with an option for a two-year extension. • The CO D-SNP program will serve Dual Eligible Members, including Full Benefit Dual Eligible, Qualified Medicare Beneficiary, and Specified Low-income Medicare Beneficiary. • The MA Plan will be responsible for coordinating the delivery of Medicaid benefits and providing coverage of Medicaid services. • The MA Plan will also be responsible for identifying and sharing information with the DHCFP to comply with CMS requirements for CO D-SNPs. • The MA Plan will provide supplemental benefits, including dental, vision, hearing aids, transportation, and personal emergency response systems. • The MA Plan will also be responsible for care coordination, including providing a case manager and facilitating Medicaid eligibility redeterminations.
