The Department of Human Services is seeking a vendor to provide barber and beauty services for the Elisabeth Ludeman Center in Park Forest, Illinois. The contractor will be required to provide haircuts, shampoo and stylings, scalp treatments, and other services as requested by the Center. The contractor must be licensed by the State of Illinois as a Barber or Cosmetologist and will be responsible for providing all necessary equipment and supplies. Services will be provided on an appointment basis, with a minimum of 9 hours per week. The contract term is expected to be from the date of the last signed signature to June 30, 2026. • The contractor will be required to provide services in a professional and workmanlike manner. • The contractor will be responsible for maintaining the work area in a manner that meets all Department of Public Health standards. • The contractor may be required to go to the houses to cut an individual's hair in certain circumstances. • The contractor will be required to submit signed Contractual Services Time Register forms for payment of services. • The contractor will be required to observe all traffic regulations and rules and regulations governing the behavior of the Center's employees.
