Description:
The State of Ohio is seeking a contractor to provide HEAP Applications for use by the Ohio Department of Development. The applications will be printed in English and Spanish, with a total quantity of 270,000. The contractor will be responsible for printing, binding, and delivering the applications to the specified location. The applications will be printed on white 60# Accent Opaque uncoated offset-smooth finish paper, with four-color process and micro-perforations. The contractor must provide proof of the applications and ensure that they meet the specified requirements. • The contractor must print 100,000 English applications for agencies, paper-banded in bundles of 50, and carton-packed 1,000 per carton. • The contractor must print 10,000 Spanish applications for agencies, paper-banded in bundles of 50, and carton-packed 1,000 per carton. • The contractor must print 160,000 English applications for OCEAN letters, carton-packed 1,000 to a carton, packed loosely, and stacks separated by chipboard. • The contractor must deliver the applications to the specified location, with inside delivery required. • The contractor must provide an affirmation and disclosure form prior to the award, which includes information about the location of services performed and state data.
