The St. Mary’s County Health Department is seeking proposals for a Behavioral Health Assisted Living Pilot program to support adults with serious mental illness. The program aims to provide assistance with daily living activities, nursing assessment, and delegation to prevent unnecessary institutionalization. Proposers must possess a license through the Maryland Office of Health Care Quality and meet the Centers for Medicare & Medicaid Services Home and Community-Based Services Setting Rules. The program will serve 4 to 6 adults with SMI, and the contractor will be responsible for providing a range of services, including care coordination, medical care, and psychiatric/mental health care. Key requirements include: • Possessing a license through the Maryland Office of Health Care Quality • Meeting the Centers for Medicare & Medicaid Services Home and Community-Based Services Setting Rules • Providing a plan for all admissions • Having an adequate disaster and relocation plan • Screening residents for risk of falls, depression, anxiety, and substance misuse • Ensuring staff are trained to understand behavioral health conditions in adults • Coordinating with the SMCHD LBHA to approve all admissions and provide care coordination services
