The Dane County Department of Human Services is seeking a provider to partner with onsite property management to offer supportive behavioral health services, including targeted case management and crisis stabilization, to residents of Enso Apartments. The provider will be responsible for delivering services to 60 studio apartments located at 715 Rethke Ave, Madison, WI 53714. The contract term is for one year, with an option to renew for four additional one-year periods. The provider will be required to meet specific performance indicators, including retaining tenancy, evaluating residents for Medical Assistance eligibility, and providing supportive services to residents. Some key aspects of the job include: • Providing supportive services and case management to Enso Apartment residents • Collaborating with property management and other stakeholders to provide comprehensive services • Developing and maintaining a resident advisory committee • Tracking service usage and achieving program goals • Maintaining adequate staffing to meet the needs of the resident population • Providing services to all 60 residents of Enso Apartments, regardless of funding source • Accessing all available funding sources to provide supportive services and case management to residents
