Description:
The State of Tennessee, Department of Finance and Administration, Division of TennCare is seeking information from vendors on their approach to recognizing primary care organizations in a Patient Centered Medical Home (PCMH) model. The PCMH program aims to improve cost, quality, and delivery of primary care to TennCare members. The program currently has 81 organizations, 492 sites, and around 650,000 attributed members. The state is looking for vendors with subject matter expertise in assessing and recognizing primary care organizations in the PCMH recognition program. Some key points to consider: • The PCMH recognition program assesses and approves primary care participants who follow a patient-centered, coordinated, and team-based approach to care. • The current recognition contains six distinct domains: Team Based Care/Practice Organization, Knowing and Managing Your Patients, Patient-Centered Access and Continuity, Care Management and Support, Care Coordination and Care Transitions, and Performance Measurement and Quality Improvement. • The state is seeking information on the vendor's approach to PCMH recognition, including their scale and scope of operations, requirements for recognition, and experience working with Medicaid plans or state agencies. • The state is also interested in learning about the vendor's ability to provide technical assistance on nationally validated quality measures and their experience with certified health claims-based validated measures.
