The Environmental Protection Agency (EPA) is seeking a vendor to provide Continuous Forms and Snap out Forms. The contractor will produce and warehouse the annual quantity of 1,000,000 Continuous Forms and 80,000 Snap out Forms. The vendor may deliver products on an as-needed basis, with a delivery fee of $1,700 plus a 4 percent bill on release fee, not to exceed $4,623.03.
