State University of New York, Downstate Health Sciences University (SUNY-DHSU) on behalf of its component hospital, University Hospital at Downstate is soliciting offers from qualified vendors (alternatively “Proposer”, “Offeror” and “Bidder”), of which one vendor will be selected to provide forty-five (45) Hemodialysis machines for SUNY DHSU’s Ambulatory Dialysis Department. The successful proposal shall include equipment, installation, training and ongoing support during the term of the contract. All systems and services must comply with applicable Federal and New York State Laws, SUNY Policies and institutional security and accessibility standards. PLEASE NOTE: Vendors must download Exhibit “B”, Mandatory Requirements and Detailed Specifications, and Exhibit “C” – Cost Proposal Grid and submit as part of a complete proposal. Any updates to the solicitation and answers to questions will be released via email. It is the responsibility of all vendors to: Register with the New York State Contract Reporter and sign up to receive notifications about this solicitation; or periodically check the New York State Contract Reporter for any updates or answers to questions pertaining to this solicitation. If interested in participating in this IFB, vendors must complete and submit their Summary Information Form (Page 1 of IFB), and submit it via email to frantz.poyau@downstate.edu. Upon receipt of the completed Summary Information Form, vendors will be added to the vendors list for this solicitation and will receive periodic updates and notifications regarding this IFB via email. Be advised that submission of responses to the solicitation that do not reflect and take into account updated information may result in your proposal being deemed non-responsive to the solicitation. This procurement is subject to NYS Finance Law Section 139 j&k, also known as the Lobbying Law. Therefore, vendors are cautioned that the only permissible contacts for this procurement are the individuals listed below. Failure to comply with this requirement can range from no contract award to debarment from sales to any agency.
