The State of North Carolina Department of Health and Human Services is seeking quotes for the acquisition of Intox EC/IR II breath alcohol testing instruments upgrade kits for the Division of Public Health, Forensic Tests for Alcohol (FTA) Branch. The kit firmware is not loaded and not used in the state network system. This purchase is for scientific component upgrades only to the actual hardware (no software). The contract term will be for one year with the option to renew for up to two additional one-year terms. • The upgrade kit includes the following components: ECIR Chassis, Case Display Bezel, Serialized Motherboard, Internal Enhancement Module, New Display with power cable, Keyboard Mini ECIR II, and USB Cable for Card Reader. • The vendor shall deliver Free-On-Board (FOB) Destination to any requested location within the State of North Carolina with all transportation costs and fees included in the total quote price. • The vendor shall provide a copy of the manufacturer's standard warranty with the quote response. • The vendor shall complete and submit the HUB Supplemental Vendor Information form to identify Historically Underutilized Businesses (HUBs) that will be used as subcontractors. • The vendor shall certify that it has the financial capacity to perform and to continue to perform its obligations under the Contract.
