OPEN-ENDED - SCHOOL BASED HEALTH CENTER SERVICES

The New York City Department of Health and Mental Hygiene is seeking qualified providers to deliver comprehensive School‑Based Health Center (SBHC) services on an open‑ended basis. The contract will cover primary medical care, mental health counseling, preventive health services, and chronic disease management for students in high‑risk NYC schools, aiming to reduce absenteeism, ER visits, and improve overall student health outcomes. Providers must have an approved HHS Accelerator PQL qualification in the PASSPort system and will submit proposals continuously through that portal. • Deliver on‑site primary care (physical examinations, immunizations, acute illness treatment) within NYC public schools. • Provide mental health and behavioral health services, including counseling, crisis intervention, and referral coordination. • Implement health education, chronic disease monitoring, and preventive programs for preschool through secondary students. • Maintain compliance with NYS Department of Health licensing, reporting, and quality‑assurance requirements. • Operate under an open‑ended, on‑going solicitation with continuous proposal submission via PASSPort.
