Chronic Disease Education and Awareness

The Centers for Disease Control and Prevention (NCCDPHP) is issuing a Notice of Funding Opportunity (NOFO) to support organizations that will strengthen national chronic disease education and awareness for chronic diseases not covered by disease‑specific CDC funding. Selected awardees (up to 10) will receive cooperative agreements ranging from $325,000 to $375,000 to implement activities such as: • Enhancing surveillance and addressing priority evidence gaps. • Developing, refining, and disseminating evidence‑based educational resources for public health professionals, health‑care providers, and the public. • Creating or improving survey tools for chronic disease assessment. • Building and strengthening strategic partnerships to broaden the reach of evidence‑based information. • Promoting adoption of evidence‑based prevention, screening, referral, treatment, disease‑management, and self‑management practices. The program aims to improve public health outcomes by increasing access to high‑quality education and data, and by fostering better use of evidence‑based practices across the chronic disease spectrum.
