Request for Proposals - Temporary Housing Prevention Program

The Oregon Department of Human Services is seeking proposals from certified Medicaid agencies to provide temporary housing prevention program services, including on-call direct support professionals (DSPs) to support individuals with intellectual and developmental disabilities (I/DD) in various Oregon counties. The selected provider will be required to provide 24-hour on-call licensed and trained DSP workers to support individuals under 18 who are at risk of temporarily having to leave their home. The contract is expected to start on June 30, 2027, with options to renew up to a cumulative maximum of 10 years. Key aspects of the job include: • Providing on-call qualified DSP staff for services in specified Oregon counties • Responding to service engagement requests and identifying DSP staff assignments within two hours of notification • Attending weekly check-in meetings and providing the assigned DSP staff schedule • Coordinating the DSP staff schedule with the contract administrator or delegate prior to providing services to an individual • Maintaining a minimum of seven qualified staff available to work within 24 hours from confirmation of the service engagement request • Ensuring all designated staff are properly trained prior to being sent on assignment • Following ODDS individualized support plans, behavior plans, medication forms, health care plans, protocols, and other approved plans • Submitting case notes, incident reports, and other reports to the community developmental disabilities program (CDDP) or ODDS as requested
