Peer-to-Peer Support Services for Families in Hawai'i County

The Hawaii State Department of Health is seeking a community-based organization to provide peer-to-peer support services to families in Hawaii County who are impacted by substance use and child welfare involvement. The services should focus on relationship-building, engagement, navigation, and practical assistance for families. The selected vendor will be required to provide services within the designated service area, establish and maintain documentation of peer support activities, and participate in coordination meetings and training. The vendor should have at least three years of experience providing services within Hawaii County, including experience working with families impacted by substance use and child welfare involvement. The scope of work includes providing peer-to-peer support services, conducting peer support contacts, and establishing and maintaining documentation of peer support activities. Key responsibilities include: • Providing peer-to-peer support services grounded in lived experience to families with young children impacted by substance use and child welfare involvement • Serving a minimum number of families, to be proposed by the vendor and approved by DOH • Assisting families with navigating services and systems, including coordination with child welfare and substance use treatment providers • Conducting peer support contacts, which may include in-person, phone-based, or virtual engagement • Establishing and maintaining appropriate documentation of peer support activities • Participating in coordination meetings, training, and technical assistance activities related to peer support service delivery
