The County of Hawaii is seeking input for the development of a Request for Proposals (RFP) for Person-Centered Case Management services for the elder population and family caregivers. The purpose of this Request for Information (RFI) is to gather community feedback and comments on the following areas: Support Plans & Expectations, Monitoring & Check-ins, Quality, Documentation & Contract Compliance, Engagement & non-compliance, and Case Management Capacity & Workforce Continuity. The RFI is open from May 6, 2026, to May 13, 2026. • Support Plans & Expectations: Describe how person-centered support plans are developed, including how program expectations are communicated to Kupuna and caregivers. • Monitoring & Check-ins: Describe the process and frequency for on-going check-ins, and how changes in needs are addressed. • Quality, Documentation & Contract Compliance: Explain how service quality is monitored, how documentation is maintained, and how adherence to contracted provisions is ensured, monitored and reported. • Engagement & non-compliance: Describe the approach to working with Kupuna and caregivers who are not meeting program expectations, including strategies to address barriers and improve participation and compliance. • Case Management Capacity & Workforce Continuity: Describe the case management staffing model, including caseload ratios and supervision, and explain the plan to maintain service continuity during staff shortages or vacancies.
