RFP Physical Therapy Services 2025-26

The Tuckahoe Union Free School District is seeking a qualified provider to deliver special‑education physical therapy services for the 2025‑26 school year. Services include individual, small‑group, and consult sessions for approximately 31 students, delivered primarily on‑site during regular school hours. The contractor must be licensed in New York, meet extensive insurance requirements, and be able to bill Medicaid and document services in the district’s IEP Direct system. Proposals must include detailed qualifications, insurance certificates, and a completed quotation sheet with hourly rates. The award will be made by the Board of Education and the contract may be extended up to five additional one‑year terms. • 37 individual 30‑minute PT sessions per week, 9 group 30‑minute sessions per week, and 3 consult sessions per month. • All therapists must be NY‑licensed, cleared under SAVE legislation, and operate as 1099 independent contractors. • Provider must supply basic therapy materials, maintain required insurance (GL, Auto, E&O, Umbrella, Workers’ Comp) and name the district as additional insured. • Documentation for Medicaid‑eligible students must be entered into IEP Direct; billing is done directly to the district.
