Health Home Performance Analysis

The State of South Dakota’s Department of Social Services is seeking a qualified vendor to perform comprehensive financial‑outcome and quality‑outcome analyses for its Medicaid Health Home (Care Connect) program. The work includes accessing historical Medicaid claims data, developing and maintaining a secure provider‑facing portal for semi‑annual outcome‑measure submissions, conducting cost‑avoidance and cost‑savings analyses using pre/post matched or alternative causal‑inference methods, and delivering both quarterly quality reports and an annual financial outcomes report. The vendor must be able to handle large CSV claim datasets, ensure HIPAA compliance via a Business Associate Agreement, and provide optional on‑site or virtual presentations as required. The contract runs for one year beginning June 1 2026, with possible annual extensions, and the total contract ceiling on the prior five‑year award was $978,574.
