Peach Springs Health Center - Propane Services

The Indian Health Service is seeking a contractor to provide continuous propane supply services for the Peach Springs Health Center and two Government Quarters locations. The contractor will be responsible for furnishing all labor, transportation, equipment, inventory management, and delivery services necessary to maintain uninterrupted propane service throughout the period of performance. The annual propane usage is approximately 5500 gallons. The contractor will need to provide a technical proposal, a price proposal, and contractor representations and certifications as part of their offer. • The technical proposal should include past performance information and technical capability. • The price proposal should include the completed, signed Attachment 1 Synopsis and the completed Schedule of Items. • The contractor representations and certifications should include a copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Products and Commercial Services.
