Description
Through this Request for Proposals (“RFP”), the Office of Temporary and Disability Assistance (“OTDA”) of the State of New York (“State”) is seeking competitive proposals from up to five (5) qualified vendors to provide Consultative Examinations (CE) for individuals referred by participating Local Department of Social Services (Districts) to obtain independent medical and psychiatric/psychological examinations and intelligence evaluations. The information provided from these examinations and any ancillary testing will be used to assist District staff in making a determination of employability under State guidelines. These examinations also may be used by Districts to make recommendations regarding referrals to the Social Security Administration (SSA) for federal disability benefits, primarily Supplemental Security Income (SSI), to determine disability for Medical Assistance (MA) recipients and/or evaluation of medical/psychological impairments affecting the ability of an individual or family to comply with an Independent Living Plan in the case of homelesshomelessness. OTDA makes no guarantee of District participation or volume of referrals.
Bids are requested for the following five regions in New York State:
Region 1 – Long Island – Nassau, Suffolk – due to the high density of the region, bidders must be able maintain at least one location in each participating District.Region 2 – Lower Hudson and Delaware Valley – Sullivan, Ulster, Dutchess, Putnam, Orange, Rockland, Westchester.Region 3 – Capital Region/North – Albany, Columbia, Greene, Schoharie, Rensselaer, Schenectady, Montgomery, Fulton, Saratoga, Washington, Warren, Essex, Franklin, Hamilton, Clinton, St. Lawrence.Region 4 – Central/North – Broome, Chemung, Delaware, Chenango, Otsego, Cortland, Tioga, Tompkins, Schuyler, Seneca, Cayuga, Onondaga, Madison, Herkimer, Oneida, Oswego, Lewis, Jefferson.Region 5 – Western – Chautauqua, Cattaraugus, Allegany, Steuben, Yates, Ontario, Livingston, Wyoming, Erie, Niagara, Genesee, Orleans, Monroe, Wayne.
Offerors may submit proposals for one or more regions. Awards will be made for each location independently. Offers cannot be contingent upon award of multiple regions. Offerors wishing to bid on more than one location must submit a separate Technical and Financial Proposal for each location bid upon.Providers must bid a percentage off the New York State Office of Temporary and Disability Assistance Division of Disability Determinations (DDD) Fee Schedule rates. See Appendix P for a list of DDD Fee Schedule prices.Office locations must be handicapped accessible and should be centrally located within each region, with public transportation and/or public parking available within a reasonable walking distance.
