Care management Services

The County of Warren seeks a contractor to provide per‑diem nursing and care‑management staff for its Aging and Disability Services Division throughout calendar year 2027. The contractor will supply registered nurses (RN) and licensed practical nurses (LPN) on a per‑diem basis to perform clinical eligibility screenings, assessments, and documentation, as well as qualified care‑managers (social workers, nurses, or bachelor‑degree holders) to conduct intake, develop individualized plans of care, coordinate services, and meet ongoing reporting and meeting requirements. The work includes PASP enrollment coordination, monthly program reporting, and adherence to state licensing, HIPAA, and insurance standards. Compensation is structured as hourly rates with ceilings of $33/hr for RN, $33/hr for LPN, and $27/hr for care managers, with bi‑weekly billing and payment within 30 days. • Provide per‑diem RN/LPN clinical services, including home visits and eligibility assessments. • Deliver per‑diem care‑management services: intake, plan of care development, monthly follow‑up, quarterly/semianual visits, and PASP enrollment support. • Maintain strict compliance with NJ licensing, HIPAA, reporting, and supervision requirements; submit bi‑weekly invoices and monthly activity reports.
