RHTP Admin: Program Evaluator

The Indiana Dept of Health is seeking a Program Evaluator for the Rural Health Transformation Program (RHTP). The primary deliverables include developing an Overall Evaluation Plan, an Annual Program Evaluation Report, and a Final Evaluation Report. The Evaluator will analyze RHTP outcome data, translate findings into actionable insights, and conduct an analysis of entities receiving RHTP funds. The Evaluator will report findings to the State Steering Committee to inform program decisions. Key aspects of the job include: • Developing an Overall Evaluation Plan based on CMS' and Indiana's RHTP goals and objectives • Developing an Annual Program Evaluation Report and a Final Evaluation Report • Analyzing RHTP outcome data and translating findings into actionable insights • Conducting an analysis of entities receiving RHTP funds • Reporting findings to the State Steering Committee
