RHTP Support of Community & Clinical Linkages

The Minnesota Department of Health seeks qualified vendors to provide statewide technical assistance to rural healthcare providers participating in the Rural Health Transformation Program (RHTP). The selected vendor(s) will support implementation of activities in the Community-Based Preventive Care and Chronic Disease Management and Sustain Access to Services to Keep Care Closer to Home areas. Key aspects of the job include: * Providing technical assistance to up to 125 rural health care organizations statewide * Supporting implementation of RHTP activities, including chronic disease prevention and management, and frontline workforce models * Providing coordinated and cohesive technical assistance in both virtual and in-person formats * Developing a plan for one-to-one technical assistance provision, collaborative networking, and written technical assistance materials * Identifying existing tools, readiness assessments, policies, procedures, guidelines, and best practices to support implementation * Providing bimonthly summaries of technical assistance requests and solutions applied * Engaging in regular collaborative partnerships with other technical assistance vendors and MDH to achieve overall RHTP goals.
