Community-Based Services

The City of Norfolk is seeking proposals from qualified offerors to provide community-based services as required by federal and state programs for children and families. The City intends to award multiple contracts for each service, and services will be purchased on an as-needed basis. The scope of services includes working collaboratively with the service coordinator and the City to ensure coordinated delivery of community-based services and maximizing state and federal funding. Some key points to note are: • The City cannot guarantee any minimum or maximum number of orders or compensation amounts. • The Contractor shall maintain an accounting system and supporting records adequate to assure that claims for funds are in accordance with applicable state, federal, and appropriate accrediting agency requirements. • The Contractor shall provide documentation of employee qualifications, experience, criminal, and child abuse record checks/results to the City upon request. • The Contractor shall participate in periodic utilization review monitoring the use and delivery of services as coordinated by the City. • The City reserves the right to determine if a proposal is incomplete, and failure to adhere to the format set forth in Section IV may result in a poor evaluation of the offeror's proposal.
