Washington County’s Behavioral Health Division is seeking a qualified provider to deliver Substance Use Disorder (SUD) community‑based outreach services to residents who are discharged from an emergency department, covering a five‑year period (July 1 2027 – June 30 2032). The provider will employ outreach specialists to meet clients in homes, shelters, vehicles, or other convenient locations, assess needs, connect individuals to treatment, housing, food, and insurance, and ensure compliance with federal confidentiality regulations. Minimum performance expectations include serving at least 100 individuals per fiscal year, delivering evidence‑based, trauma‑informed services, maintaining robust quality‑assurance processes, and submitting quarterly reports and invoices. The contract is funded by the county’s Addictions Program, with annual budgeting subject to renewal and performance.
