The Baltimore County Department of Health’s Behavioral Health Authority is seeking a qualified non‑profit organization to operate a comprehensive outreach program for individuals experiencing homelessness who have behavioral health diagnoses. The contractor will provide case management, linkage to behavioral health services, and support to achieve self‑sufficiency and permanent housing under three separate state‑funded grants (PATH, PATH‑Match, and SOAR). The scope includes outreach, assessment, service coordination, reporting, compliance with HIPAA, and meeting local minority‑business participation goals.
