Community Health Improvement Plan (CHIP): Rural Health Access and Social Supports

The Washington County Public Health (WCPH) is seeking proposals from eligible organizations to expand access to mobile healthcare services and social supports in rural Washington County communities. This project supports the implementation of the Washington County Community Health Improvement Plan (CHIP) and is funded through the Oregon Health Authority (OHA) Rural Health Transformation Program (RHTP). The selected organizations will work in partnership with WCPH to implement approved activities that support the goals of this project. The funding opportunity supports community-based approaches that expand existing mobile healthcare services in rural communities and improve connections to social supports and community resources.
