The effort is split into two optional components: (A) a three‑year pilot that provides continuous‑glucose‑monitoring (CGM) devices, integrates real‑time data with the state’s health‑information network and all‑payer claims database, delivers training and technical assistance, and reports on clinical outcomes; and (B) ongoing diabetes‑prevention and chronic‑disease management services that include evidence‑based programs, community health worker outreach, provider‑payment coordination, and technology‑enabled self‑management tools. The contract is a one‑year base term with up to four one‑year extensions (potential total of five years) and requires monthly and quarterly reporting, strict data‑residency and security compliance, and extensive insurance coverage. No dollar ceiling is disclosed; pricing must remain fixed through 30 Jun 2027.

• Component B: Diabetes prevention, DSMES enrollment, CHW outreach, and provider payment management.
• Mandatory deliverables include monthly usage reports, quarterly subcontractor spend reports, and annual impact assessments, all subject to FOIA and stringent privacy rules.
