Description
The State of Vermont is seeking a qualified organization to lead a comprehensive feasibility assessment, expansion planning process, and implementation effort to demonstrate how existing care coordination system capabilities can be extended to support a statewide interfacility transfer coordination and capacity management solution. The goal is to build stronger rural health networks, improve technology and shared operations, strengthen the rural health workforce, and ultimately ensure that Vermonters receive the right care at the right time for an affordable cost. The selected applicant will assess the ability of current care coordination infrastructure, workflows, and partnerships to be expanded and operationalized as the foundation for a statewide model. Key deliverables include a proposed statewide operating model, governance framework, technology and interoperability strategy, performance measurement framework, and a phased roadmap for expansion.
• Key aspects of the project include:
• Improving patient placement, transfer coordination, and care navigation decision-making
• Supporting real-time coordination and visibility across the health care system
• Creating a "least-call" system for providers
• Supporting more effective utilization of available capacity across Vermont's hospitals and health care organizations
• Coordinating integration with EMS resources
• Providing visibility and decision support to enable collaborative management of patient movement and system capacity
