*Proprietary* Instrument Services - LDH

The State of Louisiana, Office of State Procurement, is soliciting bids for *Proprietary* Instrument Services for the Louisiana Department of Health, Office of Public Health. The contract will be for an initial period of one year, with the option to renew for two additional 12-month periods. The successful bidder will be required to provide instrument services, including preventative maintenance visits, repair services, and technical support, for various laboratory instruments. Bidders must be OEM certified and provide discounts on training and qualification services. The bid prices must include delivery of all items F.O.B. destination, and payment will be made within 30 days after receipt of a properly executed invoice or delivery. • The contract will be awarded to the lowest responsive, responsible bidder. • The bidder must provide a bid bond, if applicable, and a performance bond, if required. • The bidder must comply with all applicable federal and state laws and regulations. • The bidder must provide a W-9 form with their bid, if newly registered. • The bidder must register in the LaGov system to submit an online bid.
