Description

The Nebraska Rural Health Transformation Program (RHTP) – Initiative 4.4b seeks to expand hospital‑to‑home Remote Patient Monitoring (RPM) capacity across rural Nebraska. Eligible Critical Access Hospitals, Rural Emergency Hospitals, Regional Rural Hospitals, and Federally Qualified Health Centers will receive cost‑reimbursable funding to procure FDA‑cleared RPM devices, build clinical workflows, integrate data into electronic health records, and train staff and patients. Applicants must submit a detailed work plan, budget, and sustainability plan, demonstrating non‑duplication of existing RPM programs and measurable outcome targets such as patient enrollment, device deployment, and readmission reduction. The award range per hospital is $200,000 to $1,000,000 with a 7.5 % indirect‑cost cap, and the performance period runs from May 12 2026 through May 11 2027. • Deploy and integrate RPM hardware/software for chronic‑disease management. • Develop and document clinical monitoring protocols, escalation pathways, and patient education. • Provide performance metrics (patient counts, device usage, readmission reductions) and a five‑year sustainability plan.

Budget estimate

$200,000 – $1,000,000

Individual awards range from $200,000 to $1,000,000, reflecting the minimum and maximum funding levels outlined in the RFA. The final amount will depend on the scope of the RPM deployment, number of devices, staff effort, and indirect‑cost cap. Thus, a winning applicant can expect to receive somewhere in that band, with larger hospital systems likely nearer the $1 M ceiling.
