Rural Health Transformation Fund Grant - Pharmacy Test-to-Treat Notice of Award

The State of Vermont is seeking grant applications from Vermont pharmacies to participate in a pilot program to expand their scope to include test-to-treat for certain conditions. The goal of the program is to enhance primary care capacity, reduce unnecessary emergency visits, and strengthen Vermont’s integrated rural health care delivery system.
