Family Navigator Services

The Washington County Juvenile Department is seeking Family Navigator services to support at-risk and underserved youth in the community. The selected provider will work closely with Juvenile Department staff to connect youth and their families with community resources, pro-social activities, and skill-building activities. The services will focus on developing positive and trusting relationships with youth and families, and will be provided in a manner that is respectful, recovery-oriented, and trauma-informed. The provider will be responsible for providing care coordination, meeting with youth and families in their community, developing trust and rapport with youth, and connecting youth and families with skill-building resources and pro-social activities. Some key aspects of the job include: • Providing care coordination and intensive case management services • Meeting with youth and families in their community to provide advocacy and support • Developing trust and rapport with youth and using motivational interviewing techniques • Connecting youth and families with skill-building resources and pro-social activities • Providing educational assistance and job skill development support • Collaborating with community-based partners to implement services tailored to meet each family's individual needs and strengths
