Chinle Nephrology Clinic Services Contract

This sole‑source modification extends the existing contract with Southwest Kidney to provide on‑site nephrology physician clinic services at the Chinle Comprehensive Health Care Facility (CCHCF) in rural Arizona. The contractor will operate four‑hour clinics on Monday and Thursday (currently) and add two additional four‑hour clinics per week, delivering outpatient nephrology care to approximately 35,000 Navajo patients. The performance period is flexible, ranging from short‑term (13 weeks) up to long‑term (1‑3 years) with possible intermittent support. Key responsibilities include patient evaluation, diagnosis, treatment planning, medical record documentation, credentialing, compliance with HIPAA and tribal cultural considerations, and rapid replacement of personnel within five days if needed. The contract requires the provider to maintain accreditation, submit accurate invoices/timesheets via IPP.gov, and adhere to extensive performance‑based metrics. • Provide on‑site nephrology physician services (4 hrs × 4 days/week, expanding to 6 days/week). • Ensure continuity of care, rapid staffing replacements, and cultural competence. • Maintain all required licenses, certifications, DEA registration, and immunizations. • Follow IHS, Joint Commission, CMS, and tribal policies for documentation and patient safety. • Submit invoices/timesheets promptly; performance evaluated via CPAR/CPARS.
