The State of Hawaii’s Department of Health is seeking a qualified vendor to provide a wide array of dental materials and supplies for its Developmental Disabilities Division under a 12‑month contract. The award will be based on the lowest total price inclusive of all taxes, fees, and shipping, submitted via the Aloha eBUYS system (Exhibit A). Vendors must be registered in Hawaii Compliance Express, carry required insurance, and submit a full price schedule for roughly 150 individual items and several generic groups (e.g., burs, impression materials). Items are to be shipped within 5 business days (or 7 calendar days) of each state purchase order, with a minimum PO value of $100. • Scope: Procurement of dental consumables, instruments, kits, and related supplies (e.g., etchants, adhesives, disposables, burs, ultrasonic inserts, X‑ray chemicals). • Contract length: 12 months, commencing Jan 2 2027; multiple repeat orders expected throughout the year. • Vendor requirements: HCE registration, $12 annual fee, CGL $1 M per occurrence ($2 M aggregate), auto liability $1 M, workers’ comp, tax‑clearance certificates, and various compliance certificates. • Evaluation: Lowest total price inclusive of taxes and all shipping/handling fees; no partial bids allowed. • Delivery: Ship via USPS, UPS, or FedEx with tracking; notify within 5 business days of any back‑orders.
