RHTP Center of Excellence - Implementation

The State of Montana’s Department of Public Health and Human Services is seeking a qualified contractor to implement the Rural Health Transformation Program (RHTP) – Center of Excellence (CoE) Implementation. The effort will provide project‑management, technical, and consulting services to help rural hospitals and clinics transition to a more outpatient‑focused, integrated care model, deploy tele‑health capabilities, support back‑office efficiencies, and coordinate inter‑facility transport systems. The contractor must deliver a comprehensive Project Management Plan, execute a multi‑year work plan, and produce regular status reports and final program‑participation documentation. • Provide full‑cycle project management (kick‑off, weekly/ monthly meetings, deliverable tracking, quality control). • Implement tele‑health, mobile care vans, facility upgrades, and an optional inter‑facility coordination system across 47 frontier counties. • Support capacity‑building, change‑management, analytics, and reporting for the CoE incentive program, ensuring compliance with federal and state regulations.
