Mental Health Case Management: Care Coordination for Children and Youth

This RFP seeks a qualified organization to provide Mental Health Case Management and Care Coordination services for eligible children, youth, and young adults (up to age 22) residing in Baltimore County. Services include comprehensive assessment, individualized care plans, coordination of medical, behavioral, educational, and social services, and ongoing monitoring to ensure continuity of care and positive outcomes.
