NNMC - HPDP Wellness Program Coordination Services

The Indian Health Service Shiprock Service Unit is seeking Indian‑owned businesses to provide comprehensive, community‑based wellness program coordination for the Northern Navajo Medical Center. The contractor will secure facilities within 25 mi of NNMC, manage equipment access, plan at least eight health‑promotion events per year, support mental‑health initiatives, deliver health‑education and culturally‑appropriate nutrition/cooking programs, provide concussion‑management training for schools, and conduct crisis‑prevention training for school staff. All services are non‑clinical and performed at contractor‑managed sites; the contract is firm‑fixed‑price with a base year and three one‑year options (total four years). • Provide exercise‑equipment and aquatic‑wellness facility access for referred IHS patients and community beneficiaries. • Plan, coordinate, and execute ≥ 8 community health‑promotion events each performance year. • Support non‑clinical mental‑health wellness initiatives. • Deliver health‑education training and supplies to students, parents, and community members. • Engage ≥ 50 unique youth participants per year in culturally‑grounded wellness activities. • Offer healthy‑living and cooking education ≥ 2 times/month focusing on heart‑healthy, diabetes‑friendly Native American cuisine. • Conduct concussion‑equipment training for local schools. • Provide evidence‑based crisis‑prevention and de‑escalation training for school staff. • Contractor must secure facility agreements, staffing, supplies, and equipment; no clinical licensure required.
