Description
The State of Wyoming, Department of Health, is seeking a proposal for the administration and management of the Rural Health Transformation Program - Make Wyoming Healthy Again Community Programming Grant Initiative. The selected proposer will be responsible for creating and administering the grant application process, evaluating applications, awarding grants, and monitoring grant recipients. The proposer will also provide technical assistance to grant recipients and maintain regular communication with the Agency regarding program performance and impact. The project term begins October 1, 2026, and the vendor must expend all funds by September 30, 2027.
