GROUP LIFE, DISABILITY, CRITICAL ILLNESS, ACCIDENT AND HOSPITAL INDEMNITY RFP

Polk County is seeking a provider for group life, disability, critical illness, accident, and hospital indemnity insurance. The selected firm will provide insured lines of coverage for all benefit plans, claims administration, employer and employee services, reporting, and online enrollment capabilities. The contract is for an initial term of three years, with the option for three successive one-year extensions. The provider must have at least 5 years of experience in providing services to similar-sized public sector employers and be able to provide insured lines of coverage for all benefit plans. • Must have 5 years of experience in providing services to similar-sized public sector employers • Must be able to provide insured lines of coverage for all benefit plans • Must provide claims administration, employer and employee services, reporting, and online enrollment capabilities • Must have a strong service team with experience in providing services to governmental entities • Must provide a detailed plan design and rates for the proposed services
