Better Choices Better Health- Ke Ola Pono Chronic Disease Self-Management Coordinator

The Hawaii County Office of Aging is requesting information from qualified applicants to provide coordination of the Better Choices Better Health- Ke Ola Pono Chronic Disease Self-Management Program. The purpose of this Request for Information (RFI) is to assist the office in obtaining community input for the development of Requests for Proposals (RFP) for coordination services. The office is seeking feedback and comments on techniques to increase recruitment and retention, community partners, experience needed for program administration, and suggestions for other services and programs for older individuals or family caregivers. * Provide information on techniques or strategies to increase recruitment and retention of participants and lay leaders for the BCBH program * Identify community partners, agencies, or stakeholders who could assist in promoting the BCBH program * Describe experience needed to successfully monitor program fidelity, oversee program administration, measure quality assurance, and identify and resolve problems within program or service delivery * Provide suggestions for other services and programs for older individuals or family caregivers that may be provided in the future
