Vision Coverage Plan Insurance

The Canyons School District is seeking a qualified firm to provide vision insurance coverage for its employees. The selected firm will be required to provide a vision coverage plan that meets the district's needs and is compliant with all applicable laws and regulations. The contract will be for a term of one year, with the option to extend for an additional four years. The district is looking for a firm that can provide a comprehensive vision coverage plan, including network provider coverage, claim processing, reporting, eligibility, and customer service. The firm must also demonstrate experience in providing similar services to large employers and have a strong local presence in the Utah community. • The firm must be properly licensed to conduct business in the State of Utah and hold all necessary certificates. • The firm must secure and maintain insurance as stated in the CSD Standard Terms and Conditions for the duration of the contract. • The firm must provide a proposal that includes a multi-year rate guarantee with at least a two-year rate guarantee. • The firm must demonstrate competence and qualifications in providing the required services. • The firm must provide a proposal that meets all administrative and technical requirements of the RFP.
