The State of Iowa, through its Department of Health and Human Services, is seeking qualified applicants to serve as pilot “Co‑Location Site Leads” for the Healthy Hometowns Rural Health Transformation Program. The selected Lead will establish up to seven rural co‑location sites that bring together at least three distinct health‑ or health‑related social‑service entities under one roof, embed Community Health Workers, and integrate services to improve chronic‑disease prevention and management. The effort is federally funded through CMS’s Rural Health Transformation Program and runs for a maximum five‑year period, with major deliverables including site renovation (limited to minor upgrades), implementation of shared telehealth and health‑information‑exchange infrastructure, development of referral and payment agreements, comprehensive reporting, and a sustainability plan for post‑grant operation. • Create and fully staff rural co‑location sites (≥ 3 service‑delivery entities, at least one providing chronic‑disease services). • Deploy ≥ 2 Community Health Workers, provide training, and coordinate care across entities. • Deliver required reporting (monthly, quarterly, annual) and meet performance milestones (sub‑contract execution, service start by Dec 31 2027). • Maintain compliance with CMS cost‑restriction rules, salary caps, and a 10 % administrative‑cost ceiling. • Develop a sustainability plan to ensure sites remain operational after the grant ends.
