A - Previous Addendum Wide-Area-Network (WAN), Leased Lit Fiber Ethernet & Internet Access Services

The University of Utah is seeking a qualified vendor to provide Wide-Area-Network (WAN) Leased Lit Fiber Ethernet and Internet Services for the Utah Education and Telehealth Network (UETN) and its constituents. The scope of work includes providing bandwidth for identified stakeholders, leveraging current UETN network resources, and meeting the requirements of the federal E-Rate discount program. Vendors must submit a proposal, cost proposal spreadsheets, and meet mandatory requirements, including compliance with the E-Rate program and registration with the Universal Service Administrative Company (USAC). • The contract term is expected to be five years with one optional year extension. • The University reserves the right to make contract awards to one or more vendors for single or multiple locations. • Vendors must provide a standard Service Level Agreement (SLA) and meet financial stability requirements. • The University is exempt from State of Utah sales and excise taxes. • Vendors must comply with applicable state and federal laws, including the Health Insurance Portability and Accountability Act (HIPAA).
